Like many people, one of your most valuable asset is your ability to earn an income. Your family's lifestyle, your home, your children's education and your retirement all hinge on your ability to work.
How would you continue to pay your bills if you suddenly became too sick or too hurt to work? Without an income, years of hard-earned savings could evaporate in a matter of months, even weeks.
That unpleasant thought is a reality that every breadwinner should consider. The odds of becoming disabled are probably much greater than you think.
About 1 out of 15 people experienced an unintentional injury in the home and community venue and about 1 out of 4,100 people died from such an injury. About 40% of the deaths and disabling injuries involved workers while they were away from work (off the job). National Safety Council, Injury Facts 2009 Edition

You may apply for monthly benefits in $100 increments from $500 up to $6,000 per month. See the Monthly Benefit Worksheet to calculate the amount of disability income insurance you're eligible to purchase.
| AGE | Monthly Benefit Amount |
|---|---|
| Under Age 50 | Up to $6,000 |
| Age 50-59 | $3,000 |
Please note: Your benefit amount is limited to 65% of your Basic Monthly Pay. Basic Monthly Pay means an Insured Person’s regular monthly rate of pay, not counting commissions, bonuses, overtime pay or any other fringe benefit or extra compensation, in effect on the last day of Active employment prior to becoming Disabled. On the Premium Due Date on or next following the date you attain age 60, your monthly benefit will reduce by 50%, with an appropriate adjustment in premium, as seen in the benefit chart above. Coverage ends at age 65. However, the maximum payment period may be reduced due to mental, emotional disorders, alcoholism or drug addiction, as specified in the Accident and Sickness Total Disability Benefit.
The benefit amount payable as the result of the Insured Person's Total Disability will be the lesser of:
- the Monthly Benefit; or
65% of the Insured Person's Pre-Disability Earnings minus:
- any Other Income Benefits, including those for which the Insured Person could collect but did not apply; and
- all other income from any employer or for any work.
However, if the Insured Person's Monthly Benefit would reduce to less than $50 per Month due to Other Income Benefits, then the minimum Monthly Benefit under The Policy will be $50 per month.

Is there a waiver of premium provision?
Yes - Future premiums will be waived beginning the first day of the month following the start of disability payments and for as long as disability benefits are payable.
What is a total disability?
- During the Elimination Period and the first 24 months during which Total Disability Benefits are payable, wholly and continuously prevents You from performing the Essential Duties of Your Occupation; and
- After that, wholly and continuously prevents You from engaging in Any Occupation.
What if I return to work after a disability and then relapse?
Because disabilities don’t always start and stop in easily defined time frames, we've developed a plan that is flexible enough to accommodate various disability durations and scenarios. If you return to work for 14 days or less during the waiting period and then relapse, you can qualify for benefits by satisfying the remainder of the waiting period. This provision relieves you of starting the entire waiting period again.
Survivor Income Benefits
If You were receiving a Monthly Disability Benefit at the time of Your death or have met the Elimination Period, We will pay a Survivor Income Benefit, when We receive proof satisfactory to Us:
- of Your death; and
- that the person claiming the benefit is entitled to it.
The Survivor Income Benefit is calculated as an amount equal to 3 times Your last Monthly Benefit Amount for Total Disability. In addition, periods of disability due to the same or related medical causes and separated by fewer than six months while you are Actively- at-Work are considered a single period of disability. This means you won’t have to satisfy a new waiting period before qualifying for benefits should you relapse upon returning to active employment after receiving benefits for a disability. You do not need to satisfy a new waiting period, the relapse does not entitle you to a new benefit period. You would only receive the remainder of the existing benefit period for that disability. Actively-at- Work is defined as performing all the regular duties of an occupation for wage or profit on a full-time basis (at least 25 hours per week).
